Create a from, customize it and add it to your site. Everytime someone uses your form their answers or text will be emailed to you. The sender will see a nice "Thank You" screen with their information on it so they can see what they sent.

How Do I Create a Form?

Click on where it says, "Create a Form", enter your email address, name you want seen on the form, subject line you want seen when the results are emailed to you, title at the top of your form, a link for your readers to go back to when their done with your form, a name for that link, a background color for your form, text color, link color for thank you page, visited link color.

How Do I Add It To My Site?

Add <form> </form> to your Web page where you want your form to show up. Then copy the form template that's given to you and paste it into your Web page code right between the "form" code you just added. Make sure all of this is inside the BODY of your Web page.
